I agree, I think he possibly just used the relationship as a crutch to get through med school/residency after realizing she wasn’t the one, but still wanted/needed her support. If he’s saying he’s not sure at this point it means he’s not sure about when to end the relationship— so he’s going to wait for OP to end it herself.
